Individual Tickets

At this time, seating assignments for the luncheon have been made; however, we have limited seating still available.  If you would still like to register, please fill out the form found here and send it to our office immediately.  (email khollingsworth@afpchicago.org or fax 630-416-9798).  Once you send in your form, please call our office at 630-416-1166 to ensure your registration was received.  Reservations will not be accepted without payment, and seating preference cannot be guaranteed.    

Tickets to the AFP Chicago 34th Annual Philanthropy Awards Luncheon can be purchased for $85 each. Tables of ten are available for $850 per table. Please include all attendee names with your registration.
